Job Details

Provide direct and indirect patient care in accordance with applicable scope and standards of practice. Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors. Ability to formulate an individualized plan of care, revise plan as indicated by the patients response to treatment, and evaluation overall plan daily for effectiveness. Monitor, record, and communicate patient condition as appropriate to care team, provider, patient, and family following chain of command as appropriate. Collaborate as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral, and spiritual/psychosocial support needs. Evaluate learning needs of patient and/or family and provide patient/family education appropriate to age, culture, condition, and circumstances.
